>Solution :
Change this line:
$insert = $conn->prepare("INSERT INTO `users`(name, email, password, image, user_type)
VALUES(?,?,?,?)");
to:
$insert = $conn->prepare("INSERT INTO `users`(name, email, password, image, user_type)
VALUES(?,?,?,?,?)");
Because you want to use 5 parameters , you also need 5 ? marks.
